perform Operational Discovery
Audits™. During these two-day onsite visits we
interview frontline staff, managers and executives to better
understand your hospital's care coordination and throughput. We
present the Operational Discovery Audit results and analysis to the
hospital team.
- We work with the hospital to determine the most desirable
future processes and create a detailed
Current-to-Future-State Analysis for patient care
and throughput.
- We collaborate with the hospital to identify and establish the
best Care Coordination Model to reach
throughput goals. This model includes job descriptions, roles and
responsibilities, and standard operating procedures for all areas
of patient care. Our transformation team remains engaged with the
hospital teams throughout the care coordination rollout.
- To help keep things on track, Care Logistics
does OnPath™ Progress Reports every six
months to ensure the teams meeting expectations and metrics for
performance.
