Care Coordination Model

It's like "air traffic control" for your patients' hospital stay...

The Care Logistics Care Coordination ModelTM is a centralized "hub and spoke" coordination center that oversees enterprise-wide patient flow 24 hours a day, 7 days a week.   By integrating departmental workflows and managing system flow, hospitals are able to optimize each patient's hospital length of stay.

The Hub

Staffed by Care Coordinators, the Hub is responsible for ensuring proper patient placement and determining initial expected hospital length of stay.  The Hub staff is also working to verify insurance and making sure all necessary payor notifications are complete.

The Spokes

Care Coordinators also work out in the various departments across the hospital.  These "spokes" are working to ensure patients flow efficiently and coordinate care with the physicians and other members of the patient care team.
